Home Health Care Eligibility
Some patients are able to have PERFOROMIST delivered to their home as part of the Medicare Part B home health care benefit. If you meet certain eligibility criteria and if the services are considered reasonable and necessary for the treatment of your illness or injury, Medicare pays for you to get certain health care services in your home, or supplies delivered to your home.35
If you qualify for this benefit, your doctor can help you find (or may recommend) a medical equipment supplier pharmacy. A medical equipment supplier pharmacy works with patients, caregivers and doctors to provide the services you need, including devices and medications.

Ask your doctor for more information about having PERFOROMIST medication and a nebulizer system delivered to your home. Or, call 1-800-MEDICARE for more information. You may also want to check with your doctor or medical equipment supplier pharmacy if they can deliver your other COPD medications and devices to your home, so you are able to obtain everything from the same source.
